Access to Health Records

Access to Records Online

You can go online and see information in your GP record, including medications, allergies, test results, immunisations and illnesses. 

You can look at your GP records on a computer, a tablet or a smartphone using a website or app. If you would like to start using online services see the  easiest way to register for this service is by downloading and using the NHS app

Anything added to your medical record from the 4th October 2023 will be visible to you automatically. If you wish to have access to your medical records prior to this date you will need to apply to the Practice for this. Click here for more information

Data Sharing

Confidential patient information is used to improve health, care and services across the NHS, for example for monitoring patient safety, developing new treatments or planning NHS services. Having data about as many people as possible ensures that NHS can make the best possible decisions.

Copy of medical records

Park Practice respects the rights of individuals to have copies of their information wherever possible.

In order for us to provide you with the information that you require, please complete the attached form and bring it to the surgery. Please be advised that it will be necessary to provide evidence of your identity (driving licence/passport/birth certificate) and address (utility bill) in person in order for us to commence with your request.

In accordance with legislation no fee will be charged for your request, unless the request is manifestly unfounded or excessive, particularly if it is repetitive. If there is to be a charge, we will contact you before any further action is taken with details of our “reasonable administrative charges” in order to comply with your request.

Proxy Access

Your surgery can grant you proxy access so that you can help another person manage their GP health and care.

A proxy may be able to act for the person they support, by:

  • ordering repeat prescriptions.
  • booking appointments.
  • contacting the surgery or speaking to surgery staff.
  • viewing test results or vaccinations.
  • accessing all or part of the GP health record, to help with health-related tasks and managing health issues.

Find my NHS number

A new service is now live to help find your NHS number. This service is for anyone living in England who has forgotten or does not know their NHS number. You can also use this service on behalf of someone else where the name, date of birth and registered home postcode is known. You can opt for the number to be sent to you by text, email or letter.
