Accessing your records online
Park Practice also offers access to health records online (immunisations, allergies, coded medical conditions, consultations dates and blood test results). If you think having access to this information would answer your query, please click on `Online Services' below and sign up for Detailed Coded Record Access. Please note that you must already have online access in order to sign up for this (booking appointments online, requesting repeat prescriptions and your repeat medication history).

Copy of medical records
Park Practice respects the rights of individuals to have copies of their information wherever possible.
In order for us to provide you with the information that you require, please complete the attached form and bring it to the surgery. Please be advised that it will be necessary to provide evidence of your identity (driving licence/passport/birth certificate) and address (utility bill) in person in order for us to commence with your request.
In accordance with legislation no fee will be charged for your request, unless the request is manifestly unfounded or excessive, particularly if it is repetitive. If there is to be a charge, we will contact you before any further action is taken with details of our “reasonable administrative charges” in order to comply with your request.
** Covid change: as we are currently only allowing access into the surgery for booked appointments, please scan/photograph your completed Subject Access Request form and email it to: firstname.lastname@example.org. Please attach a photograph of yourself holding photo ID (ideally driving licence/passport in order that we can also verify your signature) - this photo will be used for ID verification purposes only, will not be stored on any systems/medical records and will be deleted once processed. One of our team members will then call you for further identity verification. If you are looking to order medication for someone else on a proxy basis, please complete this form and return to the surgery - we will need to see ID for both parties.
National Data Opt-Out
The national data opt-out applies to the use of confidential patient information for research and planning purposes.
The national data opt-out does not apply where:
- data is shared for your individual care
- there is a risk to public health or data is required for monitoring and control of infectious diseases, for example during an epidemic
- there is an overriding public interest, for example:
- reporting of gun wounds in line with GMC guidance
- there is a legal requirement to share information, for example:
- investigations by regulators of professionals (eg General Medical Council investigating a registered doctor’s fitness to practice)
- NHS fraud investigations
- notification of food poisoning
- you have consented to take part in a specific project
- anonymised data is used.

You will need:
- your NHS number (a 10 digit number that you should be able to find on any document sent to you by the NHS; for example, a hospital referral letter or prescription)
- to have an email address or phone number registered with an NHS service.

Your individual care will not be affected if you opt out.
Confidential patient information is used to improve health, care and services across the NHS, for example for monitoring patient safety, developing new treatments or planning NHS services. Having data about as many people as possible ensures that NHS can make the best possible decisions. For example, if lots of people from a particular age group, ethnic minority or local area opt-out, then that group will not be represented in research findings. You can find out more here.
Type 1 Opt-out
Patients' personal data will be extracted and shared with NHS Digital to support vital health and care planning and research. If you do not wish to have you information shared for this purpose, please complete the form below and return to the surgery before the end of August.
Find my NHS number
A new service is now live to help find your NHS number. This service is for anyone living in England who has forgotten or does not know their NHS number. You can also use this service on behalf of someone else where the name, date of birth and registered home postcode is known. You can opt for the number to be sent to you by text, email or letter.
