Flu Vaccinations 2020 - updated 20.11.20

We can now offer a free flu vaccination to patients aged 50 and over. Please ring us to book an appointment or book online. Our first clinic is on the evening of the 2nd December. 

We are now taking booking for this years flu vaccinations. It is recommended for anyone over 65 years of age, patients with underlying health conditions and pregnant patients.  We are also taking bookings for nasal flu clinics for 2-3 year olds.  Please contact the surgery to book your appointment.

Please return this form to us by post, or scan/photograph it and send to admin.parkpractice@nhs.net

Once you have returned your forms, you can make a telephone appointment with the practice nurse to discuss your travel arrangements.  This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.
